Goal-Based Programs

Central Coast NDIS Support Services

NDIS participants often face barriers when accessing mainstream fitness options, making tailored, mobile personal training an important part of building independence and physical capacity. True Form Fitness, located in Gwandalan and servicing the Central Coast, offers specialised personal training under NDIS support services.

All sessions are delivered by a qualified trainer with a Certificate IV in Master Trainer and additional training in disability and aged care. Programs are designed to help participants achieve specific, measurable outcomes aligned with their NDIS goals, such as improved strength, mobility, confidence and daily function.

Training can be delivered in-home, outdoors, or online to suit the individual’s preferences and access needs. We work directly with participants, support coordinators and plan managers to ensure sessions are not only beneficial but also compliant with plan requirements.

Whether it’s building the confidence to navigate daily tasks or maintaining physical health through regular movement, our services are fully adaptable and person-centred. To discuss eligibility or book a session, call 0408 892 758 today.

NDIS-Ready Support

Capacity Building Through Movement

Each program begins with a consultation and basic assessment to understand the participant’s abilities, needs and plan objectives. From there, we create a structured training program that builds physical capacity in line with NDIS goals and funding categories — typically under “Improved Health and Wellbeing” or “Improved Daily Living.”

All exercises are modified to suit individual conditions and can be delivered one-on-one or in supported small groups. Our focus is on safety, accessibility and real-world improvement, with consistent tracking and communication to ensure measurable progress.

This approach supports a broad range of disabilities, including physical, intellectual, psychosocial and neurological conditions. Training may focus on strength, balance, coordination, endurance or flexibility, depending on the individual’s circumstances.

Sessions are particularly helpful for participants who want to improve independence, reduce reliance on support, or simply stay active in a safe, controlled environment. If you're looking for fitness support that aligns with your NDIS plan and personal goals, get in touch to get started.
